I declare that I did not conceal information about my health condition or the medications I am taking while filling out the form. I declare that I do not suffer from any of the following diseases: Moderate or severe hypertension or hypotension, diabetes, schizophrenia, heart disease or weakness, epilepsy and other diseases in which there is a risk of seizures, infectious or contagious diseases, suicidal tendencies, any diseases, on which the patient is taking anti-psychotics or drugs causing MAOI (monoamine oxidase inhibition). I declare that I am not a drug addict. I am aware that withholding information about my medical condition may result in injury or death. I understand that the use of pharmaceutical drugs, drugs or alcohol is not allowed to participants during the "Healing Mexico Ceremonies" stay without the prior consent of the organizers of this stay. I am aware that the use of medication may lead to the immediate termination of my participation in the stay without the possibility of compensation. I also understand that all medications must be stopped long enough before the stay that they no longer affect the normal state of the body and that there are no interactions with natural medicines, unless I am allowed to continue taking them in advance in writing after consulting with the organizers of the Healing Mexico Ceremonies stay these drugs. I am aware that discontinuing the prescribed medication should only be done after consultation with my doctor.

All activities during the stay, except herbal ceremonies or jungle trips, are considered very safe and are in the hands of experts. Even so, they carry the risk of injury to health or property, for which the organizer of the stay does not assume responsibility.
